Navigating the NDIS planning process with Capability Support Services involves five key steps to ensure individuals receive personalised support tailored to their unique needs and goals. From initial consultations to ongoing reviews, our team is dedicated to guiding you through each stage of the planning journey with care and expertise.

Step 1

Initial Assessment

This step involves determining your eligibility for the NDIS and understanding how it can benefit you. You'll provide information about your disability, its impact on your daily life, and your goals and aspirations.

Planning Meeting

Collaborate with an NDIS planner to develop a personalised plan that outlines your goals, needs, and the supports and services you require. This meeting is an opportunity to discuss your preferences and aspirations, ensuring your plan reflects your circumstances.

Plan Development

After the planning meeting, your NDIS planner will work with you to finalise your plan, ensuring it aligns with your goals and meets your needs. You'll have the opportunity to review and approve the plan before it is implemented.

Choosing Providers

Once your plan is approved, you'll be free to choose service providers who can deliver the support outlined in your NDIS plan. Take the time to research and select providers who align with your preferences and can effectively meet your needs. You will need to endorse your provider with the NDIS by calling them if you are on the new PACE system with the NDIS. You will then sign a service agreement and receive a welcome pack from your provider to commence services.

Plan Review

Regularly review your NDIS plan to ensure it continues to meet your evolving needs and goals. If necessary, you can request changes to your plan, ensuring you receive the support and assistance you require to achieve your aspirations and enhance your quality of life.
